A Beauty Dinning Club Membership online Application Form
- Applicants may provide their name and email address for membership purposes and applications may be made on site or online.
- The membership card will be issued as proof of membership upon acceptance.
- Each membership card will bear a membership number, date of membership as well as the photograph of the card-bearer.
- The membership card must be presented upon entry into the exhibition A Beauty Centre & membership number may be used to log in for updates and claim other benefits online.
- Entry to the exhibition is subject to the card-bearer’s compliance with the regulations and policies of the venue including but not limited to those relating to admission hours, dress code, prohibitions of animals, eating and drinking, smoking, photography and disruptive conduct; and admission may be refused or members required to vacate the venue on such grounds.
- Membership is accepted at the sole and absolute discretion of the card issuers and owners of the A Beauty Centre and may be revoked and terminated at our discretion.
- Membership policies, procedures and the membership card are part of the art project A Beauty Centre by artist Yeo Chee Kiong that simulates the language and appearance of beauty and aesthetic centres and does not offer, warrant or guarantee any medical, cosmetic and beauty treatments and is not a professional, medical nor clinical beauty centre and makes no claims or representations in this regard whatsoever.
- All material in A Beauty Centre is copyrighted and may not be copied, reproduced, circulated and distributed without the permission of the Artist.
- The Artist Yeo Chee Kiong, reserves the right to reproduce the membership card and to present the membership card in any contemporary art show under A Beauty Centre or any series related. A limited edition run of the membership card shall be produced and may be continued or varied at the discretion of the Artist.
Terms & Conditions
- This membership card entitles you discounts on selected merchandise and catalogues at site or online; privileges, reward or bonus points and benefits are updated on www.abeautycentre.com;
- This card is non-transferable and valid until expiry of this art project or at a date of expiry set out in our website;
- Lost, stolen and damaged membership cards may be notified to the card issuer; an administrative fee for replacement cards may be applicable;
- The card issuers reserves all rights to vary, modify and/or terminate the terms and conditions of membership.
- This card is not a credit or debit card and not proof of credit-worthiness. This card is part of the art project A Beauty Centre by artist Yeo Chee Kiong and may not be copied, reproduced and distributed without the permission of the artist;
